inTransition Program

Discussion of the inTransition program, a free DoD resource offering continuity of mental health care support for Service members, as well as Veterans, during times of transition. Specific topics to be discussed include program drivers and policy requirements, referral processes to connect patients with inTransition, coaching to care services, program limitations, recent program enhancements, in addition to transition resources available across DoD and Veterans Affairs.

Target Audience

This activity provides continuing education for physicians, nurses, social workers, and psychologists. A certificate of attendance is available for other attendees.

Learning Objectives

  1. Describe the inTransition program, to include rationale, policy drivers, and benefits for transitioning Service members (SMs) and Veterans.
  2. Identify three challenges that may arise when transitioning out of military Service, and how inTransition may assist.
  3. Utilize three best practices to enhance the likelihood that an SM or Veteran makes a successful transition to a new mental health provider in their new geographic area.
